From the School Health Clinics
If your child has any of the following medical conditions, we will need an Emergency Action Plan in place for him/her at the beginning of next school year. In cooperation with your child’s physician, please complete the action plan form that applies to your child.
Asthma Severe Allergy Seizure Bee Sting Allergy Diabetes
Your physician may have his/her own plan of care which can be utilized in place of these forms. Emergency action plans must be signed by both the physician and a parent/guardian.
To ensure the action plan is sent to all appropriate staff members prior to the first day of school, please return the form to the school as soon as it is completed and signed. Paper copies can be turned in to the main office. Copies can also be e-mailed to the appropriate school nurse at:

Any emergency medications your child requires should be turned in with the appropriate authorization forms by a parent/guardian prior to the start of school. Please note if your child will be going to 6th grade camp or a class trip, he/she must have a self-carry form completed if he/she has a rescue inhaler or EpiPen.
