7th Grade Immunization Requirements

Attention Parents of 6th graders:
In order to comply with the state of Ohio Immunization guidelines, all 7th graders must receive a Tdap and Meningitis (A, C, Y, W-135) vaccine or have a signed exemption for these vaccines prior to the 14th day of the next school year. Please see this attachment for more information- you can reply to this message if you have any questions.

Hello! This is a friendly reminder to please share your child’s updated immunization record with either the LE or LMHS clinic once they receive their required 7th grade vaccinations.
You can submit the record in any of the following ways:
Email: hoovercat@lakelocal.org
Online: Upload directly to your child’s FinalForms account
In Person: Drop off a hard copy at either LE or LMHS clinic
